11. Indicate one or more divisions: collision, mechanical, transmission.

12. Choose a catalog category for your course, from the attached list.

13. Needs Assessment: What problem or need prompted the development of this course.

14. List of what specifically the student will be able to do as a result of this course.

15. List of the person/people involved in the planning and instruction. Attach a brief bio of course instructor.

16. Detailed course outline (see attachment) and indicate where specific teaching techniques (lecture, role play, worksheets, group, etc.) that will be used.

17. Types of learning support and materials which will be provided.

18. Please enclose a copy of the workbook.

19. Please enclose a paper sample of visuals.

20. Requirements (test, attendance only, etc.), if any, for satisfactory completion.

21. What about this course is specific to the automotive service industry.

22. Name and phone number of your ASA or EXCEL 2000 referral, who is familiar with your course.
